For Active Duty You certify you are on leave/pass status at this time and will remain in when awaiting and/or having been accepted for space-A travel. If accompanied by dependents, you further certify that travel is not TDY/TAD and you are'nt using space-A to transport your dependents to/from your restricted duty station or all others (unaccompanied) tour location station. For AllBy submitting this form you certify that no element of travel is transportation via DOD-owned or controlled aircraft is for personal gain, nor for, or in connection with business of any nature and that this trip will not result in any form of remuneration to yourself or to your family. You further certify that you understand violation of any of the above could result in billing and/or punitive action.
